With Immediate Effect Passports which are issued more than 10 years ago, cannot be accepted any more. The pax will have to take a fresh passport.

Business Visa :

Valid passport last 6 months, One visa application form online filled & take back to back print, Two colour photographs 35m by 45mm 80% face should be seen white back ground , Covering letter on company letter head addressing to the Poland consulate Mumbai with company Profile, Invitation letter with the passport details of the Applicant addressing to the Poland consulate Mumbai 1 copy fax directly to the consulate & 1 copy to us , Overseas Medical Policy, air ticket, Passenger's personal IT last 3 years OR Salary slip last 3 months, Passenger's personal bank statement last 6 months, Appointment letter copy, International credit card if Passenger have.

Tourist Visa :

Valid passport last 6 months, One visa application form online filled & take back to back print, Two colour photographs 35m by 45mm 80% face should be seen white back ground , Covering letter on company letter head addressing to the Poland consulate Mumbai , Hotel confirmation from Poland 1 copy fax directly to the consulate & 1 copy to us , Overseas Medical Policy, air ticket, Passenger's personal IT last 3 years OR Salary slip last 3 months, Passenger's personal bank statement last 6 months, If Employee than required Leave letter from company, International credit card if Passenger have.

Employment visa :

Valid passport last 6 months, One visa application form online filled & take back to back print, Two colour photographs 35m by 45mm 80% face should be seen white back ground , Covering letter on company letter head addressing to the Poland consulate Mumbai , Original Work permit approval from Poland, Invitation letter from Poland company with Passenger's ppt details addressing to the Poland consulate Mumbai 1 copy fax directly to the consulate & 1 copy to us, Work experience certificate, Qualification certificate, Resume copy of applicant, Overseas Medical Policy.

W.E.F : From 2nd November, 2015 all applicants have to come personally for submission & biometrics at the consulate with prior appointment. Pls note we will make the payment & assist the applicant at the consulate.
